His history of drug abuse started more than 10 years ago, where he tries heroin, erimin, fortunately he developed severe migraine after taking these drugs, the found Dormicum as an alternative, since its easily available. What made things worst was that he works as a security guard on shifts, and that screwed up his biological clock. Hence just a pop of the blue pill, he goes into wonderland. In the span of 8-9 year, his body developed tolerance, and hence he increases the dosage till today, 30-40 per day.
15mg package.
7.5mg package.As a result of prolonged abuse, he got into fights, loss his job, was jailed for 2 months, and currently still has to go for random urine drug test.
When he came to me, he was already under follow up from HUKM, where they prescribed him valium, and antidepression to ease off his cravings, but it doesn't work.
So, by the third time he came to me for prescription, i told him frankly, that i don't just prescribe medicine indescriminately, but i ask him to think about his family, his young wife, and three young children, youngest being 1 year old. How long or what are the children's impression on him seeing his father being a drug addict, and not able to function normally? Then i told him, the next time he walks to my clinic is not for prescription, but to seek help to overcalm his addiction. The first step in treating his addiction is to admit that he has an addiction problem, and he is mentally prepared to undergo dyscomfort during the course of treatment.
So, i treated him with replacement drugs, antidepressions, additional non habit forming sleeping pills, and some drugs to control the craving.
So, my regime was. Since he is already on Lexapro(an antidepression that has the effect to controlling the cravings at night, I told him to take it in the morning, since all Antidepressions would cause insomnia! And this was prescribed to him by a profesor in psychiatry.
Seroquel, a non addicting or habit forming sleeping aid.
Rivotril, a long acting benzodiazepine that circulates in the body for 24 hours to decrease the craving for dormicum.
Zolpidem 10mg two tablets, also a sleeping tablet, to ensure he doesn't get up in the middle of then night to ask for more dormicum, which he usually does.Normally, the cocktail that i prescribed for him is enough to knock out an elephant. After about a week, he came for his weekly counseling, it appears that such drastic reduction in Dormicum made him feel incomplete! I told him he has a combination of 2 benzodiazepines, and 2 other group of sleeping pills. He insisted that he still couldn't sleep despite his wife telling me that he slept like a log. The problem is he has craving in the morning and evening, and unable to function while he's experiencing withdrawals. The following week, i added dormicum 7.5mg whenever he feels his craving starts to manifest. But instead, he took like 5-6 of 7.5mg and fall asleep instead!
I was feeling a little frustrated with his non compliance.
I tried my very best for the past 5 months. As i initially told the wife, for rehabilitation it may take about 6 months. So far, he is taking 3 dormicum 15mgs a day, but along with Stilnox and Seroquel. The other day, he came to my clinic, appear blurr, not focussed, tears were coming out from his eyes as he thinks he isn't improving and that i am beginning to feel frustrated at his progress. He had admitted that he will live and die with dormicum, and nothing is going to get rid of his craving.
That thought bothered me. I started going online searching for detox and rehabilitation programs. I have found one run by a pharmaceutical company is Taman Tun Dr. Ismail, which i wrote to them and requested to send a representative to see me and bring along brochures for me to get familiarized with the environment.
I remembered he was crying begging me not to give up on him, but i had no choice. By doing this, i don't think i am abanding him, but helped him instead.
Many doctors gave up on him, and started to talk harshly towards him, he claimed. But the comfort that he received from me needs to end if the therapy isn't fruitful.

ReplyDeleteThank you for your kind comment. Its hard to convey the message when a big man like him started crying so that i won't lose hope on him. I tried making him see what the future holds for him if he is over calm such habit of him, how respectful his children going to look upon him, his wife would have a husband to depend on. He is a good man, but trapped in this web of addiction. I am genuine in trying to help him, i even wrote to one of the hospital in US that does Detox program, inquiring how they do it, how they control the withdrawal, but so far no response. Because of his addiction, he is unemployed, and the wife has to spend at least RM 400 per week for his drugs. I made his write a diary to divert his attention, about what are his goals, how is he going to achieve it, how is he feeling about being free from addiction, yesterday, he came, he told me, he hasn't started writing yet. I pointed out to him that the diary is not just for writing, but its a commitment. I had no choice but to refer him to a detox centre. Anyway, thanks for your kind comments.

Currently i am taking up addiction cases. So, i sell them cheap but putting them on a tapering dose and prescribing them with other alternative to prevent withdrawal syndrome.
i am also taking dormicum every night. during day time iam having so much effects that troubles me iam not so much depend on dormicum.

ReplyDeleteHi,I,m a mother of 3 and I've been taking Dormicum for a year now...about 2x a week (7.5mg)if I really need to get a good night's sleep and wake up early for my kids activities. I'm a 42 year old female and I've been having trouble sleeping since I was a flight attendant 18 years ago. I used to take Melatonin and then Valium for a short while but they never worked for me so I gave them up right away.
ReplyDeleteA doctor friend prescribed Stilnox 2 years ago but I didn't like the side effects since it made me feel so groggy the whole day at school(By this time I was already an art teacher). Dormicum is the only sleeping aid I've used that gave me a really sound sleep with no side effects (like groginess).
My question is do you think that I might get addicted to this drug eventually? Lately, I've been waking up after 4-5 hours after taking it which is not enough sleep for me but I still haven't increased the dosage yet. How long and how often do you think is the safe dosage for a woman my age without getting addicted to it. Is 3 consecutive nights of 7.5mg would be considered dangerous?
One thing I have to mention after taking dormicum is that i would be feeing fresh and wide awake the whole day. Also, I tried combining it once with 1/2 a tablet of Rivotril to calm me down. Is this dangerous? Hoping to hear about your insights on this. I just dont' wanna end up an addict. Thanks a lot!

ReplyDeleteI am baffled that you mentioned about feeling grogginess after taking Stilnox while on the other hand, you felt rejuvenated with dormicum. Normally, its the other way around. Stilnox acts on a more specific part of the brain (binds specifically to the GABA receptors). It increases REM sleep, and hence even if you just slept for,say 5 hours, you should wake up feeling fresh. Wherelse, dormicum on the other hand, even it induces sleep, the quality of sleep is effected.
Initially, stilnox is being advertised as a drug that does not cause tolerance and addiction, which in my experience, is absolutely false. I have seen patient who became tolerant towards it that some may take upto 20-30 tablet at night.
The reason on why you woke up 4-5 hours after ingesting dormicum is because it is a short acting benzodiazepine, and thus, not really a drug of choice for sleep.
Are you sure the name your doctor prescribed you was Stilnox? (Zolpidem), or was it Imovane?(zopiclone)
There are many issues that we need to discuss before we jump into the topic of which drug of choice, or what duration. I have a friend who works with SIA and they were given special discount on Stilnox to readjust their sleep pattern. In fact, Stilnox is given to fighter pilots in US upon completion of a mission.

Hi, I am a recovering alcoholic trying to overcome my drinking urges. Normally I start drinking only in the evening having up to 8 units of vodka or whiskey before falling asleep.
ReplyDeleteI have started taking 5 mg diazepam in the evening around when I would drink to relax myself. I am breaking the pill into four parts and having it over a period of two hours. But in the morning I am waking up as groggy as I was when drinking. Can you recommend a sleeping aid that won't have this side effect? Thanks.

this blog really makes me cold and shock. my mother is also a dormicum dependent and now beyond control of doing irrational things. i m searching for some info that would help her,,,but it sounds like it is almost impossible to cure her,,,:((

ReplyDeleteI totally disagree. Even heroin addicts can be salvaged and had their life turned around and today has become important member of society. I personally know doctors, counselors, millionaires, successful motivational speakers who was once heroin addicts.
